Cmoy Headphone amplifier

Just finished with my Cmoy headphone amplifier inspired from Headwize.com.
Chu Moy made a very basic and simple headphone amplifier based on OPA2134 op-amp by Texas Instruments.

I was having OPA4134 so thought of using the 2 spare channels for a line out, may be to drive another pair of headphones.
Apart from running headphones crystal clear, another advantage is that it can act as gain enhancer for my gainclone when watching movies from PC. here are the pics.

What voltage in? 12v?
Im not very good at electronics.

anything between 8-18 volts. i designed it to Run on 9 volt battery, but currently running on 18 volts AC adapter..
